For any changes to your demographics or insurance, please contact our office to update your information as soon as possible.

Scheduling An Appointment

Appointments are available Monday through Thursday 8:00 AM – 11:30 AM and 12:45 PM – 4:15 PM.

The office is closed for lunch from 12:00 – 12:45 PM.

Friday 8:00 AM – 11:45 AM (The office is closed after 12:00 PM).

Patients should arrive 30 minutes prior to their scheduled appointment if they have not completed the initial registration online. If you do not register for our patient portal, you may print and complete forms by going to our Documents & Forms page.

Please Bring The Following With You To EVERY Appointment:

  • Insurance Card (Please note: If patient is not the primary policy holder we will need the full name and date of birth of the policy holder)
  • Co-Pay/Deductible/Co-Insurance will be collected at the time of service
  • Photo Identification
  • Medication names and dosages or the bottles *Please include over the counter medications and supplements
  • Name and phone number of preferred pharmacy
  • Name and phone number of referring practice/provider
  • Name and phone number of primary care provider (if different from referring practice/provider) – this will ensure good communication between our office and your other healthcare providers

Canceling An Appointment

We understand that there will be instances where scheduled appointment times cannot be kept. However, please remember to call and cancel your appointment.  Failure to do so prevents other patients from being seen.  Our policy requires 24 hours’ notice. Our office will charge $100.00 fee if you fail to show up to your scheduled appointment or fail to provide 24 hours’ notice when cancelling an appointment.  Three consecutive “NO SHOWS” may jeopardize your future appointment availability and is subject to a discharge from our practice.

Laboratory Testing

Laboratory facilities are conveniently located in our office and are offered by Quest Diagnostics. These services are provided separately and independently from our practice. Therefore, any services or billing issues with labs will need to be directed to Quest Diagnostics. Their customer service number is (866) 697-8378.

Prescription Requests

Call your pharmacy and ask them to fax refill requests to our office at (919) 571-3290.  DO NOT wait until you are out of medicine to request a refill. Please note that refill requests may take 24 to 48 business hours. You may also use our portal to request prescription refills.

Test Results

Please allow up to 7 business days for our office to contact you regarding your test results.

Medical Records Request

You may print and complete an Authorization To Release and/or Request Medical Records Form by going to our Documents & Forms page.

After Hours And Weekends

Call 911 or go directly to the emergency room designated by your insurance company.  Request endocrine services at hospital check-in. We do not provide after-hours coverage in our office.
